With EZGuard form management, you can edit existing forms and create new forms

Where I can see which forms I have?
Enter EZGuard website
In the menu on the left side, scroll to the Management menu
Choose "Forms" button
Here you can see all your forms you have
How to edit a form?
Enter EZGuard website
In the menu on the left side, scroll to the Management menu
Choose "Forms" button
Next to the form name, there is an "Edit" button
As you click on that, an edit window will be open
When finished editing, click on "Save" at the bottom of the page
How to download form's questions list as PDF?
Enter EZGuard website
In the menu on the left side, scroll to the Management menu
Choose "Forms" button
Next to the form name, there is a 3 dots button
As you click on that, a drill down list will be open
There will be a "Download form's question list as PDF" button
As you click on that, a PDF file will be downloaded to your "Download" folder in your computer
How to duplicate a form?
Enter EZGuard website
In the menu on the left side, scroll to the Management menu
Choose "Forms" button
Next to the form name, there is a 3 dots button
As you click on that, a drill down list will be open
There will be a "Clone form" button
As you click on that, an edit window will be open. You will just need to give a new name to the form
When finished editing, click on "Save" at the bottom of the page
How to see a form that been inactivated ?

In the menu on the left side, scroll to the Management menu
Choose "Forms" button
Under the column "Active" the default that you see is "Yes". You can click on "Yes" and change to all or no and you will see the inactive forms
Where can I see to which sites the form is assign to?

How to export forms data?

As you click on that, there will be an "Excel – Raw Data" button
As you click on that, an Excel file will be downloaded to your "Download" folder in your computer
How to change the form name?

Next to the form name, there is an "Edit" button
As you click that, an edit window will be open
Under the title "General details" there is a "Name" write box
You can change the name of the form
When finished editing, click on "Save" at the bottom of the page
How to inactive a form?

Next to the form name, there is an "Edit" button
As you click that, an edit window will be open
Under the title "General details" there is an "Active" switch
Turn the switch "Off" to inactive the form
When finished editing, click on "Save" at the bottom of the page
How to enable upload an image to a form from phone gallery?

Under the title "App Configuration" the is a "Phone Gallery Enable" switch
Turn the switch on / off to active or inactive

How to schedule form report sending?

As you click that, an edit window will be open
Under the title "Report Settings" there is a "Notification" title
Next to that, choose "Scheduled"
You will have to choose the sending time and you can choose to get the forms in a grouped report
When finished editing, click on "Save" at the bottom of the page
How to set immediate notification form sending?

How to change the default question layout in a form group report?

You can choose between horizontal and vertical

How to change a logo for a form report?

How to include images that was added to the form to the email report?

How to add a question to a form?

In the menu on the left side, scroll to the Management menu
Choose "Forms" button
Next to the form name, there is an "Edit" button
As you click that, an edit window will be open
Choose "Questions, Answers and Titles" category
At the bottom of the page there is an "+Add Question" button
As you click on that, an edit widow will be open
Add the question details and choose the question type:
Free Text – the answer will be a free text
Number – the answer will be numbers only
Single Choice – the answer will be between multiple option and can choose only one
Multi Choice – the answer can be multiple option
Date – the answer will be a date
Time – the answer will be time
Date & Time – the answer will be a date and time
Image – the answer will be to take a picture
Scale – the answer will be to choose a number on a scale
Signature – the answer will be to add a signature
Barcode – the answer will be to scan a barcode and the system will add the QR code number to the answer
At the right side of the window, you can set
Required – the question will be required to answer. Can't complete the form without answering the question
Hidden in Reports – the question won't be showed in reports
Note Required – must to add a note to the answer
Image Required – must to add an image to the answer
When finished editing, click on "Save" at the bottom of the window
If you finished editing all your form, click on "Save" at the bottom of the page
How to edit a question in a form?

Choose "Forms" button
Next to the form name, there is an "Edit" button
As you click that, an edit window will be open
Choose "Questions, Answers and Titles" category
Next to the question you want to edit, the is an "Edit" button
As you click on that, an edit window will be open
When finished editing, click on "Save" at the bottom of the window
After that, click on "Save" at the bottom of the page
How to change the question order in a form?

How to delete a question in a form?

How to assign a form to a site?

Next to the form name, there is an "Edit" button
As you click that, an edit window will be open
Choose "Assignments, Recipients and Ownership" category
Under the title "Assignment and recipients" you can choose which site will be using this form by mark the checkbox next to the site name
When finished editing, click on "Save" at the bottom of the page
How to remove a site from a form?

Choose "Forms" button
Next to the form name, there is an "Edit" button
As you click that, an edit window will be open
Choose "Assignments, Recipients and Ownership" category
Under the title "Assignment and recipients" you can remove the mark at the checkbox next to the site name
When finished editing, click on "Save" at the bottom of the page
How to change the owner of a form?

In the menu on the left side, scroll to the Management menu
Choose "Forms" button
Next to the form name, there is an "Edit" button
As you click that, an edit window will be open
Choose "Assignments, Recipients and Ownership" category
Under the title "Form Ownership" there is a "Change current owner" title
You can choose this title and type a new name / email for ownership
When finished editing, click on "Save" at the bottom of the page
How to set global recipients for form?

Next to the form name, there is an "Edit" button
As you click that, an edit window will be open
Choose "Assignments, Recipients and Ownership" category
Under the title "Global Recipients" you can add recipients by choosing "+Add" button
