EZGuard Management - Forms

With EZGuard form management, you can edit existing forms and create new forms

Management - Forms

  1. Where I can see which forms I have?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Here you can see all your forms you have

  2. How to edit a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click on that, an edit window will be open

    6. When finished editing, click on "Save" at the bottom of the page

  3. How to download form's questions list as PDF?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is a 3 dots button

    5. As you click on that, a drill down list will be open

    6. There will be a "Download form's question list as PDF" button

    7. As you click on that, a PDF file will be downloaded to your "Download" folder in your computer

  4. How to duplicate a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is a 3 dots button

    5. As you click on that, a drill down list will be open

    6. There will be a "Clone form" button

    7. As you click on that, an edit window will be open. You will just need to give a new name to the form

    8. When finished editing, click on "Save" at the bottom of the page

  5. How to see a form that been inactivated ?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Under the column "Active" the default that you see is "Yes". You can click on "Yes" and change to all or no and you will see the inactive forms

  6. Where can I see to which sites the form is assign to?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Under the column "Assignments"

  7. Where can I see if the form is assigned to a checkpoint?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Under the column "Checkpoints"

  8. Where can I see if the form has a global recipient?

  9. Where can I see when the form was created?

  10. Where can I see the notification settings choose for a form?

  11. Where can I see who is the owner of a form?

    1. Enter EZGuard website

  12. How to add new form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. On the top right corner, there is a "+Add" button

    5. As you click on that, an edit window will be open

    6. When finished editing, click on "Save" at the bottom of the page

  13. How to export forms data?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. On the top right corner, there is an "Export Data" button

    5. As you click on that, there will be an "Excel – Raw Data" button

    6. As you click on that, an Excel file will be downloaded to your "Download" folder in your computer

  14. How to change the form name?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "General details" there is a "Name" write box

    7. You can change the name of the form

    8. When finished editing, click on "Save" at the bottom of the page

  15. How to inactive a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "General details" there is an "Active" switch

    7. Turn the switch "Off" to inactive the form

    8. When finished editing, click on "Save" at the bottom of the page

  16. How to enable upload an image to a form from phone gallery?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "App Configuration" the is a "Phone Gallery Enable" switch

    7. Turn the switch on / off to active or inactive

    8. When finished editing, click on "Save" at the bottom of the page

  17. How to add form summary screen in a form?

    1. Enter EZGuard website

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "App Configuration" the is a "Form Summary Screen" switch

    7. Turn the switch on / off

    8. When finished editing, click on "Save" at the bottom of the page

  18. How to add "Report by" field in a form?

    4. Next to the form name, there is an "Edit" button

    6. Under the title "App Configuration" the is a ""Report By" Field Required" switch

    7. Turn the switch on / off

    8. When finished editing, click on "Save" at the bottom of the page

  19. How to add form signature required ?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "App Configuration" the is a "Form Signature Required" switch

    7. Turn the switch on / off

    8. When finished editing, click on "Save" at the bottom of the page

  20. How to schedule form report sending?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "Report Settings" there is a "Notification" title

    7. Next to that, choose "Scheduled"

    8. You will have to choose the sending time and you can choose to get the forms in a grouped report

    9. When finished editing, click on "Save" at the bottom of the page

  21. How to set immediate notification form sending?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "Report Settings" there is a "Notification" title

    7. Next to that, choose "Immediate"

    8. When finished editing, click on "Save" at the bottom of the page

  22. How to set a minimum score for sending form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "Report Settings" there is a "Notification Minimum Score" title

    7. Next to this title, there is a writing box, there you can write the minimum score for sending

    8. When finished editing, click on "Save" at the bottom of the page

  23. How to change the default question layout in a form group report?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "Report Settings" there is a "Group Report Default Questions Layout" title

    7. You can choose between horizontal and vertical

    8. When finished editing, click on "Save" at the bottom of the page

  24. How to enable map in form report?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "Report Settings" there is a "Enable Map in reports" title with a switch button

    7. Turn the switch on / off

    8. When finished editing, click on "Save" at the bottom of the page

  25. How to enable logo in form report?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "Report Settings" there is a "Enable Logo in reports" title with a switch button

    7. Turn the switch on / off

    8. When finished editing, click on "Save" at the bottom of the page

  26. How to change a logo for a form report?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "Report Settings" there is an "Override Logo for reports" title

    7. Next to the title, there is a "Select Image" button to add a logo for the form

    8. When finished editing, click on "Save" at the bottom of the page

  27. How to include images that was added to the form to the email report?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "Report Settings" there is a "Include images as attachments in email" title with a switch button

    7. Turn the switch on / off

    8. When finished editing, click on "Save" at the bottom of the page

  28. How to active auto "New" statues in form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Under the title "Status Management" there is an "Auto "New" Statues" title with a switch button

    7. Turn the switch on / off

    8. When finished editing, click on "Save" at the bottom of the page

  29. How to add a question to a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Choose "Questions, Answers and Titles" category

    7. At the bottom of the page there is an "+Add Question" button

    8. As you click on that, an edit widow will be open

    9. Add the question details and choose the question type:

      1. Free Text – the answer will be a free text

      2. Number – the answer will be numbers only

      3. Single Choice – the answer will be between multiple option and can choose only one

      4. Multi Choice – the answer can be multiple option

      5. Date – the answer will be a date

      6. Time – the answer will be time

      7. Date & Time – the answer will be a date and time

      8. Image – the answer will be to take a picture

      9. Scale – the answer will be to choose a number on a scale

      10. Signature – the answer will be to add a signature

      11. Barcode – the answer will be to scan a barcode and the system will add the QR code number to the answer

    10. At the right side of the window, you can set

      1. Required – the question will be required to answer. Can't complete the form without answering the question

      2. Hidden in Reports – the question won't be showed in reports

      3. Note Required – must to add a note to the answer

      4. Image Required – must to add an image to the answer

    11. When finished editing, click on "Save" at the bottom of the window

    12. If you finished editing all your form, click on "Save" at the bottom of the page

  30. How to edit a question in a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Choose "Questions, Answers and Titles" category

    7. Next to the question you want to edit, the is an "Edit" button

    8. As you click on that, an edit window will be open

    9. When finished editing, click on "Save" at the bottom of the window

    10. After that, click on "Save" at the bottom of the page

  31. How to change the question order in a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Choose "Questions, Answers and Titles" category

    7. Next to each question there is arrow buttons that you can click on to move up and down

    8. When finished editing, click on "Save" at the bottom of the page

  32. How to duplicate a question in a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Choose "Questions, Answers and Titles" category

    7. Next to the question you want to duplicate, there is a 3 dots button

    8. As you click on that, a drill down list will be open

    9. Choose "Clone"

    10. An edit window will be open

    11. When finished editing, click on "Save"

    12. After that click on "Save" again at the bottom of the page

  33. How to delete a question in a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Choose "Questions, Answers and Titles" category

    7. Next to the question you want to duplicate, there is a 3 dots button

    8. As you click on that, a drill down list will be open

    9. Choose "Delete"

    10. When finished editing, click on "Save" at the bottom of the page

  34. How to add a title in a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Choose "Questions, Answers and Titles" category

    7. At the bottom of the page, there is a "+Add Title" button

    8. As you click on that, an edit window will be open

    9. When finished editing, click on "Save"

    10. After that, click on "Save" again at the bottom of the page

  35. How to edit title in a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Choose "Questions, Answers and Titles" category

    7. Next to the title there is a "Edit" button

    8. As you click on that, an edit window will be open

    9. When finished editing, click on "Save"

    10. After that, click on "Save" again at the bottom of the page

  36. How to change the place of a title in a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Choose "Questions, Answers and Titles" category

    7. Next to the title there is arrow buttons that you can click on to move up and down

    8. When finished editing, click on "Save" at the bottom of the page

  37. How to delete a title in a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Choose "Questions, Answers and Titles" category

    7. Next to the title there is an "Edit" button

    8. As you click on that, an edit window will be open

    9. At the edit window there is a "Delete" button

    10. When finished editing, click on "Save" at the bottom of the page

  38. How to assign a form to a site?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Choose "Assignments, Recipients and Ownership" category

    7. Under the title "Assignment and recipients" you can choose which site will be using this form by mark the checkbox next to the site name

    8. When finished editing, click on "Save" at the bottom of the page

  39. How to remove a site from a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Choose "Assignments, Recipients and Ownership" category

    7. Under the title "Assignment and recipients" you can remove the mark at the checkbox next to the site name

    8. When finished editing, click on "Save" at the bottom of the page

  40. How to change the owner of a form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Choose "Assignments, Recipients and Ownership" category

    7. Under the title "Form Ownership" there is a "Change current owner" title

    8. You can choose this title and type a new name / email for ownership

    9. When finished editing, click on "Save" at the bottom of the page

  41. How to set global recipients for form?

    1. Enter EZGuard website

    2. In the menu on the left side, scroll to the Management menu

    3. Choose "Forms" button

    4. Next to the form name, there is an "Edit" button

    5. As you click that, an edit window will be open

    6. Choose "Assignments, Recipients and Ownership" category

    7. Under the title "Global Recipients" you can add recipients by choosing "+Add" button

    8. When finished editing, click on "Save" at the bottom of the page
